Program Guidelines

  • This is a dental discount plan and not dental insurance.
  • It cannot be used in conjunction with another dental plan or dental insurance.
  • Family members cannot be substituted for other family members.
  • Enrollees must be stated at the time of enrollment.
  • Discounts do not apply to take home whitening products.
  • Annual enrollment fee must be paid in full on or before first dental visit.
  • This plan is NON-REFUNDABLE and is for one year from acceptance date.
  • Any unused benefits cannot be carried over to another year.
  • Patient's portion of bill is due the day of service.
  • Failure to pay at time of service, failed or broken appointments without a 24-hour notice, voids your plan.
  • Plan and enrollment fees are subject to change annually without notice.
  • If using CareCredit, 0% interest plan does not apply.
